MY POWER OF SPORT CONTEST (the "Contest")

Rules and Regulations

How to Enter:

No purchase is necessary in order to participate in the Contest.
Contest starts at 12:01 a.m. Pacific Standard Time ("PT") on August 18th, 2009 and closes at 11:59 p.m. PT on September 25th, 2009 (the "Contest Period").
First entries may be made by logging on to www.livethepowerofsport.ca during the Contest Period, successfully completing the Contest entry form and telling us a story of how you LIVE the Power of Sport. In no more than 150 words tell us about lifestyle changes you have made or activities you have added that have had a positive impact on your life and/or the lives of others around you. Tell us how this has contributed to your improved level of physical and mental wellness. To be eligible for the Contest, all fields must be completed in full on the Contest /Profile entry form. Entries that do not contain a valid email address will be void. Entries that contain false, misleading or inappropriate content may be disqualified at the sole discretion of the Contest Sponsors (as defined below).
Only one entry per person during the Contest period is permitted.
All entries become the property of The Forzani Group Ltd. ("FGL") and Sport Chek International 2000 Ltd. ("Sport Chek") (FGL and Sport Chek collectively, the "Contest Sponsors"), and will not be returned or acknowledged.
Entries must be received within the Contest Period to be eligible. The sole determinant of the time for the purposes of a valid online entry in this Contest will be the Contest server machine(s). The Contest Sponsors will not be responsible for an entrant's inability to get through on the internet during the Contest Period or for entries that are lost, misdirected or delayed.
By submitting a Contest Entry Form, entrants agree to be bound by these official rules and regulations and all decisions of the Contest Sponsors.

Who Can Enter:

The Contest is open to all residents of Canada aged 18 years and over at the time of entry (except residents of Quebec and employees of the Contest Sponsors and affiliated companies, advertising and promotional agencies, and all family members of such persons and persons with whom these employees are domiciled). The Contest is subject to all federal, provincial and municipal laws and is void where prohibited.

Grand Prize:

On or about September 28th, 2009 in Calgary, Alberta, one (1) entrant (the "Selected Grand Prize Entrant") from all eligible entries received during the Contest Period will be selected by the Selection Committee to win the Grand Prize (as defined below). The Selection Committee will select the Selected Grand Prize Entrant baed upon what it believed, in its sole discretion, to be the most inspirational sports story about the entrant, has the power to motivate and inspire others, has had a positive impact on your life and/or the lives of others and best exemplifies the Live the Power of Sport concept. The "Grand Prize" includes the opportunity to appear in a Sport Chek advertisement and a $2,000 Sport Chek Gift Card. The Grand Prize must be accepted as awarded, may not be sold, exchanged, transferred or refunded and is not convertible to cash. The Contest Sponsors reserve their right to substitute a prize or prize component with one of equal or greater value.

Weekly Prizes:

At the end of each week during the Contest Period, a random draw will be made from all eligible entries received since the beginning of the Contest Period (the "Weekly Draw"). Each selected entrant of a Weekly Draw (the "Selected Weekly Entrant used interchangeably with the Selected Grand Prize Entrant") who complies with all paragraphs of these Rules and Regulations (the "Weekly Winner" and collectively with the Grand Prize Winner as the "Winner(s)") will receive a $100.00 Sport Chek Gift Card (the "Weekly Prize")to be used to by the Weekly Winner to purchase product from any Sport Chek store.
Approximate retail value of each Weekly Prize is $100.00. (the "Weekly Prize" and used interchangeably with the Grand Prize as the "Prize"). Each entrant will be eligible to win only one Weekly Prize. Each Weekly Winner will still be eligible to win the Grand Prize.

Grand Prize Winner Selection and Weekly Prize Draw:

Within five (5) business days following each of the Weekly Draws and the Grand Prize selection, the Selected Entrant will be contacted by e-mail at the e-mail address stipulated on their respective Contest entry form as submitted and given a prize notification ("Prize Notification").
If the Selected Entrant does not respond to the Prize Notification within five (5) business days thereof or does not comply with these official rules and regulations, the Prize which they have been selected for will be automatically forfeited and another entrant will be chosen.
Return of any Prize Notification as undeliverable may result in disqualification and an alternate entrant may be selected by random draw.
The odds of being chosen as a Selected Entrant depend upon the number of eligible entries received.

How to Win:

To be declared a Winner and recipient of the Prize, the Selected Entrant must first (a) correctly answer, unaided, a time-limited (1) mathematical skill-testing question to be administered by the Contest Sponsors at a prearranged mutually convenient time, (b) sign a declaration confirming compliance with the official rules and regulations and acceptance of the Prize as awarded, (c) sign and return, within the time stipulated by the Contest Sponsors, a Waiver of Liability releasing the Contest Sponsors and their respective officers, directors, shareholders, volunteers, governors, employees, agents, representatives, parents, successors, affiliates, subsidiaries, assigns, related entities, partners, partnerships, licensees, and licensors from any liability in connection with this Contest, the entrant's participation therein and/or the acceptance, possession and use of the Prize or any portion thereof, and (d) sign and return, within the time stipulated by the Contest Sponsors a Publicity Release form. By entering this Contest, the Winner authorizes the Contest Sponsors to broadcast, publish and disseminate his/her name, address, photograph, likeness, sobriquet and voice, in connection with any further publicity carried out in connection with this Contest, at no additional compensation to the Winner beyond the awarding of the Prize.
The Selected Entrant may also be required to submit valid proof of identity. If the Selected Entrant is unable to provide valid proof of identity, he or she may, at the sole discretion of the Contest Sponsors, be disqualified.

Disclaimer and Liability Exclusion:

The Contest Sponsors are not responsible for late, lost, incomplete, illegible or misdirected entries or for any failure of the web site during the Contest; for any technical malfunction or other problems relating to the telephone network or lines, computer on-line systems, servers, access providers, computer equipment or software, failed network connections, failed, incomplete, garbled or delayed computer transmissions, online failures or communication failures other circumstances affecting, disrupting or corrupting the Contest in any manner, including without limitation, receipt of entries, entrants' ability to participate in the Contest, prize notification or entrant response to the notification, for any reason including, but not limited to, technical problems or traffic congestion on the Internet or at any web site, or any combination of the foregoing. Further, the Contest Sponsors are not responsible for any damage to entrant's or third party's computer occasioned by participation in the Contest or by downloading any information necessary to participate in the Contest.
The Contest Sponsors reserve their right to cancel, terminate, modify, amend or extend this Contest or modify the official rules and regulations at any time without notice and shall have no liability to anyone, including entrants, for the cancellation or termination of the Contest.
The Contest Sponsors shall take all reasonable steps to attempt to deliver the Prize to the Winner within 90 days of the satisfaction of the requirements by the Winner. Should the Contest Sponsors be unable to deliver the Prize to the Winner within 60 days due to factors outside of the control or power of the Contest Sponsors, the Contest Sponsors shall have no liability in contract or tort to any person, including the Winner, for such delayed delivery.
By entering the Contest each entrant agrees that the Contest Sponsors shall not be liable for losses or injuries of any kind resulting from acceptance or possession or use of any prize awarded and entrants agree to release, discharge and hold harmless the Contest Sponsors, their parents, subsidiaries, successors, assigns, affiliates, licensors, agents, employees, representatives, officers, directors or shareholders, for all claims or damages arising out they will take no action against the Contest Sponsors, their parents, subsidiaries, successors, assigns, agents, affiliates, employees, representatives, officers, directors or shareholders for any loss or damage resulting from the Winner's participation in the Contest or acceptance, possession or use of the Prize.
The Contest Sponsors make no warranty, guarantee or representation of any kind concerning the Prize and disclaim any implied warranty.

General Conditions:

The Contest is managed by the Contest Sponsors. All decisions of the Contest Sponsors are final and binding on all entrants without right of appeal.
Personal information provided by each entrant by entering the Contest becomes the property of the Contest Sponsors. For more information please visit the Privacy & Security Policy found online at www.sportchek.ca.
By entering this Contest, each entrant consents to the Contest Sponsors' collection, use and disclosure of information on the Contest entry form both for the purposes of administering this Contest and, in the event the entrant opts in, to send sportchek.ca promotional and/or sportchek.ca newsletters to the entrant.
As a condition of participating in the Contest, each entrant agrees that any dispute or cause of action arising out of or connected with the Contest shall be resolved individually, without resort to any form of class action proceeding, and exclusively before a court located in the province of Alberta and that the court shall apply the laws of the province of Alberta doctrines of conflict of law and that you agree to be subject to personal jurisdiction in the province of Alberta. Further, each entrant agrees to waive all rights to claim punitive, incidental or consequential damages, or any other damages, including legal costs or attorneys' fees, other than the entrant's out-of-pocket expenses associated with entering the Contest.
In the event of a dispute, online entries will be deemed to have been submitted by the authorized account holder of the email address submitted at the time of entry. "Authorized account holder" is defined as the person who is assigned an email address by an internet provider, online service provider, or other organization (e.g. business, educational institute etc.) that is responsible for assigning email addresses for the domain associated with the submitted email address. If the identity of an online entrant is disputed, the authorized account holder associated with the email account at the time of entry will be deemed to be the entrant.
CAUTION: Any attempt to deliberately damage any web site or undermine the legitimate operation of the Contest is a violation of criminal and civil laws and should such an attempt be made, the Contest Sponsors reserve the right to seek damages or other remedies from any such person(s) responsible for the attempt to the fullest extent permitted by law.
The name of the Winner may be obtained by calling Sport Chek Customer Service at 1-877-977-CHEK (2435) within 120 days of the Contest closing date.
